Please fill out and submit the Student Enrollment Form.   You can bring the form into the office, or fax it to us.  In addition to the Student Enrollment Form, you will need to come into the office to fill out the Emergency Contact Card, and give us copies of your childs Birth Certificate and Shot Records.

There are still seats available for most grades, though space is limited.  To secure your childs spot we need to receive the signed  Student Enrollment Form. We then need you to come into the office to finalize the additional paperwork.
